As a clinician you can focus on:
- Your relationship with your patients
- Assessing for your patients’ emotional health
- Tying in to other screens and assessments
- Supporting your patients’ relationship with their caregivers
- Providing relational health interventions and referrals when needed
As a practice you can:
- Make time: Consider existing workflows and how your team can provide different activities that promote patients’ relational health by considering how the work fits in with what you are already doing.
- Connect to resources: Most communities have existing networks, formal and informal, you can connect your patients to for the concrete and community supports they need after they leave your office.
